ObjectivesThe study aimed to analyse the association between Sports-Related Concussion (SRC) and Subsequent Musculoskeletal Injury (MSK) in United Kingdom university-aged rugby union players whilst considering the effects of sex, athlete playing position and injury location.DesignRetrospective cohort study. A period of 365 days with 0–90, 91–180 and 181–365 days sub-periods was analysed for the following variables; MSK injury incidence, occurrence, severity, injury location, playing position and sex.SettingInjury data was collected from the Sports Development Centre database at Loughborough University.ParticipantsA total of 408 injuries in 181 athletes (55 females and 126 males) were included.ResultsThe MSK injury incidence of SRC group was significantly higher than control and higher post-SRC than pre-SRC period over a 365-day period (p=0.012 and p=0.034, respectively). The odds ratios of MSK injury incidence between groups and between periods were 1.62 (95% CI, 1.10–2.25) and 1.57 (95% CI ,1.08–2.29). A SRC was not associated with a greater time loss from a subsequent MSK injury or a specific MSK injury location.ConclusionsAthletes with a second recorded injury were more likely to sustain a MSK injury if they had experienced SRC, however, there was no indication a SRC resulted in greater time loss from a MSK injury.  相似文献

Background

Early interdisciplinary rehabilitation (EIR) in neurointensive care is a limited resource reserved for patients with moderate to severe traumatic brain injury (TBI) believed to profit from treatment. We evaluated how key parameters related to injury severity and patient characteristics were predictive of receiving EIR, and whether these parameters changed over time.

Methods

Among 1003 adult patients with moderate to severe TBI admitted over 72 h to neurointensive care unit during four time periods between 2005 and 2020, EIR was given to 578 and standard care to 425 patients. Ten selection criteria thought to best represent injury severity and patient benefit were evaluated (Glasgow Coma Scale, Head Abbreviated Injury Scale, New-Injury-Severity-Scale, intracranial pressure monitoring, neurosurgery, age, employment, Charlson Comorbidity Index, severe psychiatric disease, and chronic substance abuse).

Results

In multivariate regression analysis, patients who were employed (adjOR 1.99 [95% CI 1.41, 2.80]), had no/mild comorbidity (adjOR 3.15 [95% CI 1.72, 5.79]), needed neurosurgery, had increasing injury severity and were admitted by increasing time period were more likely to receive EIR, whereas receiving EIR was less likely with increasing age (adjOR 0.97 [95% CI 0.96, 0.98]) and chronic substance abuse. Overall predictive ability of the model was 71%. Median age and comorbidity increased while employment decreased from 2005 to 2020, indicating patient selection became less restrictive with time.

Conclusion

Injury severity and need for neurosurgery remain important predictors for receiving EIR, but the importance of age, employment, and comorbidity have changed over time. Moderate prediction accuracy using current clinical criteria suggest unrecognized factors are important for patient selection.  相似文献

目的分析长臂单轴椎弓根螺钉经 Wiltse入路在单纯性胸腰椎压缩性骨折中的应用效果及对肌肉损伤的影响。方法选取自 2018年 1月至 2020年 8月周口市中心医院收治的单纯性胸腰椎压缩性骨折病人 87例,根据复位固定术式不同,分为观察组 38例、对照组 49例,其中观察组采取长臂单轴椎弓根螺钉经 Wiltse入路,对照组采取 AF系统经传统后正中入路。记录两组病人围术期指标、手术前后血清肌肉损伤指标[肌酸激酶( CK)、肌红蛋白( Mb)]变化;采用疼痛视觉模拟评分( VAS)评价病人不同时间节点疼痛水平;经胸腰椎正侧位片检查获取伤椎原始高度( H0)、术前及术后 1周伤椎椎体前缘高度( H1H2),计算术前、术后 1周伤椎相对高度( H1/H0、H2/H0)及伤椎复位率,并测量矢状位 Cobb角变化;随访 3个月,采用 Oswestry功能障、碍指数( ODI)评价功能障碍情况,并统计手术相关并发症。结果观察组手术时长、术中失血量、术后引流量及首次下床活动时间均明显短于或低于对照组( P<0.05)。术后 3d,两组血清 CK、Mb水平均较术前升高( P<0.05),且观察组血清 CK、Mb水平  相似文献

BackgroundMachine learning has been applied to improve diagnosis and prognostication of acute traumatic spinal cord injury. We investigate potential for clinical integration of machine learning in this patient population to navigate variability in injury and recovery.Materials and methodsWe performed a systematic review using PRISMA guidelines through PubMed database to identify studies that use machine learning algorithms for clinical application toward improvements in diagnosis, management, and predictive modeling.ResultsOf the 132 records identified, a total of 13 articles met inclusion criteria and were included in final analysis. Of the 13 articles, 5 focused on diagnostic accuracy and 8 were related to prognostication or management of traumatic spinal cord injury. Across studies, 1983 patients with spinal cord injury were evaluated with most classifying as ASIA C or D. Retrospective designs were used in 10 of 13 studies and 3 were prospective. Studies focused on MRI evaluation and segmentation for diagnostic accuracy and prognostication, investigation of mean arterial pressure in acute care and intraoperative settings, prediction of ambulatory and functional ability, chronic complication prevention, and psychological quality of life assessments. Decision tree, random forests (RF), support vector machines (SVM), hierarchical cluster tree analysis (HCTA), artificial neural networks (ANN), convolutional neural networks (CNN) machine learning subtypes were used.ConclusionsMachine learning represents a platform technology with clinical application in traumatic spinal cord injury diagnosis, prognostication, management, rehabilitation, and risk prevention of chronic complications and mental illness. SVM models showed improved accuracy when compared to other ML subtypes surveyed. Inherent variability across patients with SCI offers unique opportunity for ML and personalized medicine to drive desired outcomes and assess risks in this patient population.  相似文献

目的 探讨S100蛋白(S100 protein,S100)水平联合Rotterdam计算机X线断层扫描(Computed tomography,CT)评分、格拉斯哥昏迷评分(Glasgow coma scale,GCS)在创伤性颅脑损伤(Traumatic brain injury,TBI)病情和预后不良评估中的价值。方法 回顾性分析106例TBI患者的临床资料,比较不同病情TBI患者血清S100水平、Rotterdam-CT评分,分析血清S100水平与Rotterdam-CT评分、GCS评分的相关性; 根据患者预后情况分为预后良好组和预后不良组,比较2组性别、年龄、血清S100水平、Rotterdam-CT评分、GCS评分等临床资料,多因素logistic回归分析TBI患者预后不良的相关因素; 分析S100蛋白水平、GCS评分、Rotterdam-CT评分及三者联合应用对TBI患者预后不良的预测价值。结果 轻度组、中度组、重度组血清S100水平、Rotterdam-CT评分逐渐增高(P<0.01),TBI患者血清S100水平与GCS评分(r=0.396,P=0.001)、Rotterdam-CT评分(r=0.289,P=0.002)均呈正相关; 2组血氧饱和度、GCS评分、呼吸频率、Rotterdam-CT评分、S100蛋白水平、入院时昏迷占比等指标有明显差异(P<0.05或P<0.01); 多因素logistic回归分析显示呼吸频率、血氧饱和度、入院时昏迷占比、GCS评分、Rotterdam-CT评分、S100蛋白水平均为TBI患者预后不良的相关危险因素; 受试者工作特征曲线(Receiver operator characteristic curve,ROC)显示S100蛋白水平、GCS评分、Rotterdam-CT评分对TBI患者预后不良均有一定的预测价值,三项指标联合应用曲线下面积(Area of the under curve,AUC)值大于各单项指标预测。结论 S100蛋白水平、GCS评分、Rotterdam-CT评分是TBI患者预后不良的相关危险因素,S100蛋白水平联合GCS评分、Rotterdam-CT评分在TBI患者预后不良评估中具有较高的临床价值。  相似文献

背景与目的: 早期乳腺癌保乳术后辅助大分割放疗已被指南推荐,但大分割放疗同期瘤床加量是否可行目前尚无定论。本随机对照研究对大分割同期瘤床加量和常规分割同期瘤床加量两种放疗计划的心脏剂量学参数和心超指标等进行比较,以评估前者在心脏毒性方面的安全性。方法: 纳入复旦大学附属中山医院2017年3月—2018年3月收治的符合入组标准的早期乳腺癌保乳术后左侧乳腺癌患者40例,随机分为两组,20例制定大分割同期瘤床加量放疗计划(全乳放疗剂量4 005 cGy/15次,同期瘤床加量至4 500 cGy/15次),20例制定常规分割同期瘤床加量放疗计划(全乳放疗剂量5 000 cGy/25次,同期瘤床加量至6 000 cGy/25次),比较两组患者的心脏剂量学参数和心脏超声检查指标,剂量学参数包括全心脏、左心室、右心室、右室游离壁、室间隔和冠状动脉左前降支(left anterior descending,LAD)的平均剂量(Dmean)以及各结构受到5~35 Gy照射的相对体积分数(V5 Gy~V35 Gy)。心超随访指标包括主动脉根部直径、左房内径、左室舒张末内径、左室收缩末内径、室间隔厚度、左室后壁厚度、肺动脉收缩压、左室射血分数、E峰、A峰、E峰减速时间(E-peak deceleration time,DT)、E、A、E/A和S波峰值。此外,对两组放疗后乳房的美容效果也进行评估。结果: 大分割组和常规分割组全心脏的Dmean分别为(471.86±170.54)和(733.07±79.11)cGy(P<0.05),全心脏的V20 Gy为(3.63±1.74)%和(8.43±0.74)%(P <0.05),V30 Gy为(1.55±1.15)%和(4.48±1.01)%(P<0.05),LAD的Dmean分别为(1 250.17±600.33)和(1 847.20±933.23)cGy(P>0.05),左心室的Dmean分别(908.64±865.60)和(946.93±116.13)cGy(P>0.05),右心室的Dmean分别为(590.37±197.99)和(905.73±180.82)cGy(P<0.05),右室游离壁的Dmean分别为(939.40±284.23)和(1 597.30±446.31)cGy(P <0.05),室间隔的Dmean分别为(637.49±248.19)和(988.60±159.77)cGy(P<0.05)。随访1年,大分割组与常规分割组相比,心超指标均在正常区间,差异均无统计学意义(P>0.05)。两组美容效果也无显著差异(P>0.05)。结论: 大分割同期瘤床加量放疗计划较常规分割计划可以显著降低全心脏和部分心脏亚结构的受照剂量,随访1年心超指标无异常,临床采用本研究所用的剂量分割方式是安全可行的。仅限制全心脏剂量无法有效保护LAD,建议勾画心脏亚结构并单独限制剂量以更好地保护心脏。  相似文献

The aim of this paper was to evaluate the association between ‘asymptomatic or mildly symptomatic’ severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection (AS/MS-COVID) and surgical site infection (SSI) after repair of craniomaxillofacial injury (CMFI). Using a case-control study design with a match ratio of 1:4, we enrolled a cohort of AS/MS-COVID cases with immediately treated CMFI during a one-year period. The main predictor variable was SARS-CoV-2 infection (yes/no), and the outcome of interest was SSI (yes/no). The other variables were demographic, clinical, and operative. Appropriate statistics were computed, and p<0.05 was considered statistically significant. The study group comprised 257 cases (28.8% female; 13.2% aged ≥ 60 years; 10.5% with fractures; 39.7% with involvement of nasal/oral/orbital tissue [viral reservoir organs, VROs]; 81.3% with blunt trauma; 19.1% developed an SSI [vs 6.8% in the control group]) with a mean (SD) age of 39.8 (16.6) years (range 19–87). There was a significant relation between SARS-CoV-2 infection and SSI events (p<0.0001; odds ratio 3.22; 95% confidence interval 2.17 to 4.78). On subgroup analysis, SSIs significantly increased with age ≥ 60 years, presence and treatment of fracture, contact with VROs, and prolonged antibiotic use (PAU). However, multivariate logistic regression analysis confirmed a positive effect only from old age, contact with VROs, and PAU (relative risk = 1.56, 2.52, and 2.03, respectively; r = 0.49; p = 0.0001). There was a significant 2.8-fold increase in SSIs among AS/MS-COVID cases, especially in those aged ≥ 60 years, or those with injuries to VROs, or both, who therefore required PAU.  相似文献
